Women`s Health Practice -
Pessare

Pessaries are valuable medical aids in modern pelvic floor therapy. They are used for organ prolapse, incontinence and to support involution after pregnancy.

They come in different shapes, sizes and materials. They are inserted and worn vaginally.

Terri Robertson Elder compares pessaries to a "sports bra for pelvic organs." Pessaries provide support, stabilize the bladder neck and vaginal wall, and place the organs in a better position. This allows the tissues to regenerate, the pelvic floor muscles to work more efficiently, and incontinence or descent symptoms to be significantly alleviated in everyday life and during sports.

Pessary consultation

To whom do I recommend the pessary consultation?

A passar is used as a supportive device for incontinence, lowering, and to assist with involution.

Due to its position and shape, it stabilizes the vaginal walls and bladder neck and returns the organs to their original position. This allows tissues and structures to regenerate and the pelvic floor muscles to work efficiently again. There are pessaries that can be worn when needed (e.g., only when jogging) and others that can remain in the vagina for several months (e.g., in cases of high-grade prolapse).

I recommend the Passar consultation to all women who suffer from complaints such as incontinence or lowering or who want to optimally support their postpartum period.

The 60-minute session includes, but is not limited to:

  • detailed discussion of acute problems and wishes
  • Extensive explanation of the anatomy and function of the pelvic floor and organs.
  • vaginal examination of the pelvic floor
  • Assessment of a POP (pelvic organ prolapse).
  • Examination of the core muscles (abdomen, back, diaphragm)
  • Assessment of pelvic statics and loading of the feet, knees, hips in stance and gait.
  • Assessment of posture and joint mobility
  • Demonstration of the handling and mode of action of a pessary

Afterwards you will receive, among other things:

  • individualized training plan taking into account your lifestyle and goals
  • individualized treatment plan depending on the symptoms

Pessary consultation with fitting

To whom do I recommend the pessary consultation?

Pessaries are valuable medical aids in modern pelvic floor therapy for organ prolapse and incontinence.

I recommend the pessary consultation to all women who suffer from stress incontinence or organ prolapse. A pessary can also be worn for support during the postpartum recovery period.

Depending on the severity of the diagnosis and the individual needs of the woman, the pessary can be worn daily or load-dependent (e.g. only during sports). After professional instruction and fitting, the pessary can be inserted, removed and cleaned independently.

What can I expect during the pessary consultation?

The 90-minute session includes, among other things:

  • Examination of the pelvic floor muscles
  • Explanation of the anatomy of the pelvic organs and the functional disorders
  • Presentation of the pessary types and their respective advantages
  • Try on different sizes
  • practice the correct handling
  • Movement tests with pessary inserted
  • Education about handling, care, storage
  • Information on the preparation of the vagina for pessary therapy (moisturizing, fat or estriol ointments).

(The 60min session is for general information about pessaries and their effect and application. There is no pelvic floor examination or pessary trial in this hour).

Pessary control appointments

After we have found a pessary that is suitable for you and you use it during physical activities or permanently, control appointments take place at intervals.

These serve to:

  • adjust the size of the pessary (e.g. in the regression period you need gradually smaller sizes)
  • control the surface of the pessary
  • examine the vaginal mucosa
  • Discuss vaginal care (moisturizers, lipids, or estriol ointments).
  • assess any side effects (discharge, feeling of pressure)
  • Clarify questions and uncertainties
